It's actually my favourite of the four films. I saw it at the cinema and loved it. The upsetting violence inflicted on the Karen villagers is remedied by the justice that Rambo brings to the Burmese soldiers. You get so pissed off at the extreme violence at the start of the film, and it's such a release when John finally unleashes mayhem on them.
Like you said, the plot isn't amazing but really a complex plot isn't necessary for this film. Not every film needs some crazy mind-bending plot.
It would have been easy to write off this film, after all it is a 60+ year old man playing an action character that he last played when he was in his 30s or so, and it is a movie franchise that hadn't been touched for a good while so it could have ended up being nothing more than a lame attempt at reprising a classic series of films. Thankfully, in my opinion, it was just as good as the other three if not better.

I heard rumours but I doubt it. It wouldn't really make sense considering the ending of the fourth film. Stallone doesn't seem the type to just cash in, so if the story is wrapped up, it'd be strange of him to put another out there.

This and First Blood are definitely the two best.

It's still amazing what First Blood managed to accomplish with its body count of 1 and that guy died by accident

I've heard about Rambo 5 too.
If they are making one I would definitely go see it.
I won't believe it at least not 100% until Sly himself says so.
